 Advance Medical Directive

An advanced medical directive allows you to name an individual as your agent and authorizes your agent to make broad health care decisions for you if you are unable to do so. The advance medical directive is especially important to avoid disputes within a family as to who is authorized to make difficult medical decisions and ensure that your wishes are known regarding life terminating decisions.

State Forms
Each state has a state specific advance medical directive. It is a good idea to have a living will and advance medical directive in the state your currently are stationed in and the state of your home of record, as each state's document may have different statutory requirements.
